Stages of dental surgery

Consultation: dentist and patient looking at an X-ray on screen — illustration for the section "How an apicoectomy is performed"
Step 01

How is an apicoectomy performed?

An apicoectomy involves several stages: diagnosis with a CT scan, anesthesia, access to the root through a gum incision, removal of the inflammatory focus and the root apex, root canal filling, and suturing of the wound. The procedure is performed under a microscope.

Preparation: the assistant lays out sterile instruments, the doctor puts on gloves — illustration for the section “How long does recovery take after bone grafting”
Step 02

How long does recovery take after bone grafting?

After bone grafting, recovery takes from a few days to a week: swelling subsides and the wound heals. Full integration of the bone material occurs over several months, which is taken into account when planning implantation.

The treatment stage itself: the doctor's gloved hands working in the oral cavity — an illustration for the section "Preparation for surgery"
Step 03

Preparation for surgery

Before the procedure, an examination is performed, an X-ray or CT scan is taken, and anesthesia is selected. The doctor explains the stages of the procedure and gives preparation recommendations, such as refraining from food for several hours if general anesthesia is planned.

Follow-up visit: the dentist checks the result with a mirror while the assistant holds the saliva ejector — illustration for the section "Rehabilitation after tooth extraction"
Step 04

Recovery after tooth extraction

After tooth extraction, it is important to follow the recommendations: do not rinse the mouth during the first 24 hours, avoid hot food, and take the prescribed medications. A follow-up examination is scheduled after a few days.

Who is a surgeon and how do they differ from a general dentist?

In dentistry, doctors' work is divided by tasks. A general dentist treats the tooth from the inside, while a surgeon addresses problems requiring intervention in the tissues. Understanding the difference helps you choose the right specialist.

Responsibilities of the surgeon and the general dentist

SpecialistMain tasksExamples of procedures
General dentistTreatment of caries and its complicationsFillings, root canal treatment
Oral surgeonSurgery on oral tissuesTooth extraction, apicoectomy
General dentistRestoration of tooth shape and colorRestorations, veneers
Oral surgeonPreparation for prostheticsBone grafting, sinus lift
General dentistDiagnosis and preventionExaminations, hygiene
Oral surgeonTreatment of inflammationAbscess drainage, cyst removal

When you are referred to a surgeon

A general dentist treats a tooth if it can be saved. When the destruction is extensive, the root is damaged, or an inflammatory focus has formed around it, surgical intervention is required. A referral to a surgeon is given for periostitis, acute periodontitis, cysts, and before implantation. The doctor assesses the clinical picture and decides whether surgery is needed. Sometimes the surgeon works together with the general dentist: for example, when extracting a tooth that cannot be treated conservatively. A surgeon is also consulted for complicated wisdom tooth eruption — gum inflammation or an improperly positioned tooth. In each case, the decision is made by the doctor after examination and diagnostics.

Can a surgeon place fillings?

The question of whether a surgeon can place fillings arises in patients when a general dentist is unavailable. The answer depends on how work is organized in a particular clinic and on the specialist's qualifications.

Separation of specialties

In dentistry, there is a clear separation between the general dentist and the surgeon. The general dentist treats caries and its complications and restores teeth with fillings. The surgeon performs tooth extractions, periodontal tissue surgery, and implantation. These are different fields requiring different skills and instruments. A doctor who has specialized in surgery does not always have sufficient practice in therapeutic treatment. Therefore, in most clinics, fillings are placed by the general dentist, while the surgeon performs surgical procedures. This separation allows each specialist to improve in their field and provide quality treatment.

Combining roles in the clinic

In some clinics, especially small ones, doctors combine roles. This is possible if the specialist holds certifications in both specialties and regularly pursues continuing education. For example, a dentist who performs extractions may also treat cavities if they have the appropriate training. However, this is more the exception than the rule. When choosing a clinic, it is worth clarifying who will provide the treatment. If a filling is needed, it is better to see a general dentist. Our clinic employs doctors of various specialties, and responsibilities are distributed in accordance with standards. The decision about who will treat a tooth is made by the doctor after an examination, based on the clinical picture.

What is bone grafting and when is it needed

Bone grafting is the surgical restoration of lost jawbone tissue. The procedure creates a foundation for subsequent implant placement or prosthetics. The need for it is determined by the doctor after diagnostics.

Indications for bone grafting

IndicationDescription
Bone atrophyReduction in bone volume after prolonged absence of a tooth
TraumaBone defect resulting from a fracture or injury to the jaw
Cyst or tumorRemoval of a neoplasm that required bone resection
PeriodontitisInflammation that led to resorption of bone tissue around the teeth
Developmental anomalyCongenital underdevelopment or deformity of the jaw
Preparation for implantationInsufficient bone volume for implant placement

Types of bone grafting

  • Guided bone regeneration: a membrane is used that isolates the defect and stimulates the growth of the patient's own bone. This is a method in which conditions are created for bone regeneration within a specified volume.
  • Autografting: transplantation of the patient's own bone tissue, for example, from a section of the lower jaw. This material takes well, but an additional procedure is required to harvest the bone.
  • Allografting: use of donor bone tissue that has undergone special processing. This avoids additional trauma, but the material may be rejected.
  • Xenografting: use of bone material of animal origin, most often bovine. The material serves as a scaffold for new bone, but the rate of regeneration may be lower.
  • Synthetic materials: bone substitutes based on hydroxyapatite or other biocompatible substances. They are accessible and safe, but require precise matching of the size and shape of the defect.

What is frenuloplasty

Frenuloplasty is a procedure to correct the frenulum of the lip or tongue. The frenulum is a thin fold of mucosa that restricts tissue movement. The intervention helps eliminate problems with speech, bite, and gums.

Indications for surgery

  • Short lingual frenulum: restricts mobility and impedes the pronunciation of sounds, especially in children. Early intervention helps avoid speech therapy problems.
  • Upper lip frenulum: provokes diastema — a gap between the front teeth. Frenuloplasty is often performed before orthodontic treatment.
  • Gum recession: the frenulum pulls the gum margin, exposing the tooth root. This causes sensitivity and increases the risk of root caries.
  • Difficult prosthetics: the fold interferes with the fixation of removable dentures. Eliminating the pull improves the fit of the denture and patient comfort.
  • Orthodontic treatment: the pull of the frenulum shifts teeth after bite correction. Therefore, frenuloplasty is often performed before or during the wearing of braces.
  • Gum inflammation: tension of the frenulum aggravates periodontitis. Surgical intervention helps reduce inflammation and improve hygiene.

How the procedure is performed

The procedure is performed under local anesthesia. The surgeon makes an incision, excises or repositions the frenulum, then places sutures. The intervention lasts 15–30 minutes. A scalpel or laser is used. Laser technique reduces bleeding and shortens healing time, but the choice of method depends on the clinical picture. After the procedure, sutures are placed; they either dissolve or are removed after a week. Recovery takes several days: swelling and discomfort subside, but final tissue healing takes up to a month. Rehabilitation includes a soft diet and oral hygiene. The doctor provides recommendations and schedules a follow-up visit. The result is assessed after a few weeks, once the tissues have fully healed. The procedure is performed on an outpatient basis, and you can go home afterward.

How is gum recession treated?

Gum recession is the lowering of the gum margin, which exposes the tooth root. Treatment depends on the degree and cause. The doctor selects the method after examination and diagnostics.

Methods for covering recession

  • Conservative therapy: in the early stage, professional cleaning, removal of dental deposits, and anti-inflammatory treatment are performed. This stops progression but does not restore tissue.
  • Surgical flap: the surgeon elevates a mucoperiosteal flap, repositions it, and secures it with sutures. This method is suitable for minor recession, when the tissue can be stretched over the root.
  • Free gingival graft: a piece of connective tissue is taken from the palate and sutured to the area of recession. This increases gingival thickness and covers the root.
  • Collagen matrix: a membrane is used instead of the patient's own tissue. This is less traumatic, but the result depends on the clinical picture and the size of the defect.
  • Microsurgical technique: using a microscope and fine instruments allows more precise work with tissues. The choice of method is up to the surgeon after assessing the periodontal condition.

Prognosis and limitations

The prognosis for recession treatment depends on several factors: the depth of the defect, the condition of the bone tissue, the thickness of the gum, and the cause of the recession. If the recession is caused by improper brushing, hygiene is corrected first; otherwise, the result will not be stable. With orthodontic treatment, the recession may be covered after the bite is corrected. Surgery is not performed in the presence of active inflammation or poor oral hygiene. In some cases, complete root coverage is impossible, especially with pronounced bone atrophy. In that case, the goal is to stop the recession and prevent sensitivity. The doctor honestly assesses the chances and proposes a realistic plan. How long the result lasts depends on care and regular check-ups. Following recommendations reduces the risk of recurrence but does not eliminate it entirely.

Is a home visit by a surgeon available?

Surgical intervention is usually performed in the clinic. But there are situations when it is difficult for the patient to get around. Let's look at whether a doctor's home visit is possible.

Possibility of a home visit

A surgeon's home visit is not a standard service but rather an exception. In most dental practices, including ours, surgical procedures are performed in the office. The reason is simple: safe treatment requires special equipment, sterile instruments, and the ability to monitor the patient's condition. However, in some cases, for example, when a person is unable to visit the clinic after surgery or due to a serious illness, the doctor may make a home visit for an examination and consultation. Such a visit does not replace full treatment, but it allows the situation to be assessed and decisions to be made about further steps. If you specifically need surgical intervention, it is better to come to the clinic, where all the conditions for high-quality work are available.

Limitations of the home setting

Home conditions impose serious limitations. A surgeon cannot perform a complex operation, since this requires an assistant, sterile drapes, a surgical suction device, and other equipment. Even a simple procedure, such as tooth extraction, can be risky: in the event of a complication, immediate assistance will be needed, which cannot be provided at home. Therefore, a house call is usually limited to an examination, suture removal, or dressing change. The decision on whether a visit is possible is made by the doctor after speaking with the patient. The doctor assesses how safe it is to perform the procedure outside the clinic. If the situation cannot wait and the patient cannot move, a visit for an emergency consultation is sometimes arranged. But planned treatment is always carried out in a dental office.

Equipment for surgical interventions

Surgical treatment requires precision. The equipment in the office determines how predictably the intervention will proceed. We will describe the main instruments and diagnostic technology.

Main instruments

  • Scalpel: disposable stainless steel blades. Used for incising soft tissue. They provide a precise cut without damaging adjacent structures.
  • Periosteal elevator: an instrument for elevating the periosteum. Available in various sizes and shapes. It allows careful separation of tissue from bone.
  • Forceps and elevators: for extracting teeth and roots. The set includes various modifications. The choice depends on the position of the tooth and its condition.
  • Surgical motor: a handpiece with a physiodispenser. It allows osteotomy and trepanation. It provides precision work with bone tissue.
  • Scissors and needle holders: for working with soft tissues and placing sutures. Instruments must be sharp and comfortable for the surgeon.
  • Coagulator: for stopping bleeding. It is often part of combined devices. It allows controlling hemostasis during surgery.

Diagnostic equipment

Accurate diagnosis is necessary before the intervention. X-rays provide a general idea, but for complex cases a cone beam computed tomography scanner is used. It creates a three-dimensional model of the jaw, making it possible to assess the position of the roots, nerves, and sinuses. A microscope with 25× magnification helps to see the smallest structures. The 3Shape intraoral scanner replaces impressions: it digitizes the dental arch without discomfort for the patient. A laser is used for cutting and coagulating tissue. Class B autoclaves sterilize instruments after each patient. Diagnostic technology reduces risks and makes treatment more gentle.

Preparing for Surgery

Surgery requires preparation. How the procedure and recovery go depends on it. We explain what needs to be done in advance.

Examination and tests

  • Surgeon consultation: the doctor assesses the condition of the teeth and gums, reviews the medical history, and determines the scope of the intervention. Discuss all your questions at the appointment.
  • X-ray diagnostics: a periapical or panoramic image — to assess the roots, bone tissue, and position of adjacent teeth. This is a necessary planning step.
  • Blood test: complete blood count and clotting test to rule out the risk of bleeding during surgery. Results are valid for several days.
  • Allergy history: clarify whether there are reactions to anesthetics or antibiotics in order to choose safe medications. This reduces the risk of complications.
  • ECG and consultation with a general physician: for chronic heart or vascular disease, a conclusion on the admissibility of the intervention is required. The doctor will assess the risks.

Recommendations for the day before

Avoid alcohol and smoking for 24 hours before surgery — they reduce blood clotting and slow healing. Do not take aspirin or other blood-thinning medications unless approved by your doctor. You may eat one hour before the procedure, but it is best to avoid a heavy dinner the night before. Arrive at the clinic early to fill out paperwork and prepare calmly. Bring your ID and a list of medications you take. If a tooth extraction is planned, ask whether you need preventive antibiotics — your doctor will decide. Do not drive after surgery, so arrange for someone to accompany you.

What should be done after a sinus lift?

Recovery after a sinus lift takes several weeks. The outcome depends on following your doctor's instructions and on individual factors. Below are general recommendations that help reduce risks.

The first days after surgery

  • Apply cold: ice in a cloth bag on the cheek on the side of the surgery for 10–15 minutes every hour during the first day. This reduces swelling.
  • Take prescribed medications: antibiotics and painkillers are taken strictly according to the regimen given by the doctor. Missing a dose increases the risk of inflammation.
  • Sleep on a high pillow: the head should remain elevated both in the first days and during the first week. This reduces pressure in the sinus area and decreases swelling.
  • Eat carefully: food should be soft and warm. Chew on the healthy side, avoid hot and cold dishes so as not to irritate the surgical area.
  • Do not blow your nose or sneeze with your mouth closed: sudden pressure in the nasal cavity can displace the graft. When sneezing, keep your mouth open.
  • Maintain hygiene: brush your teeth with a soft brush, avoiding the surgical area. Do not rinse your mouth in the first days so as not to wash out the blood clot.

Prohibitions and restrictions

  • Do not blow your nose for 2–3 weeks: this creates excess pressure in the sinus. If you need to blow your nose, do it gently, one nostril at a time.
  • Do not drink through a straw: the suction creates a vacuum and can disrupt healing. Drink from a glass in small sips.
  • Do not fly during the first month: pressure changes during a flight are dangerous for the sinus lift area. Postpone flights until you have fully recovered.
  • Do not lift heavy objects or bend over suddenly: physical exertion increases intracranial pressure. You can return to sports after 3–4 weeks, but only with your doctor's permission.
  • Do not smoke or drink alcohol: nicotine constricts blood vessels and slows healing, while alcohol increases swelling and can interfere with the effect of medications.
  • Do not visit a swimming pool, sauna, or steam bath: they carry a higher risk of infection, and overheating increases swelling. These restrictions apply until the stitches are removed.

How to choose a good surgeon

The choice of surgeon determines the outcome of the procedure. The doctor assesses the risks, plans the stages, and performs the surgery. Let's go over the criteria and questions for a consultation.

Selection criteria

  • Education and certifications: a degree in oral and maxillofacial surgery and regular continuing education courses. This confirms their theoretical training.
  • Experience: the number of procedures performed, especially complex ones — wisdom tooth removal, bone grafting, implant placement. Practice is important for confidence.
  • Specialization: a surgeon who works with a specific type of procedure every day performs more confidently. Ask which procedures the doctor performs most often.
  • Equipment: a microscope, CT scanner, laser — having this technology reduces risks and speeds up recovery. Modern equipment indicates that the clinic is serious.
  • Patient reviews: real stories about the course of treatment and the result, not just a rating on the website. Read independent platforms.
  • Personal contact: the doctor explains the plan, answers questions, and does not make impossible promises. Trust in the doctor is an important part of treatment.

Questions at the consultation

At your appointment, ask how many similar surgeries the surgeon has performed in the past year and what complications have occurred. Find out what type of anesthesia is planned and how long the procedure takes. Ask to see the images or a CT scan — the doctor can use them to explain the anatomy and the risks. Ask about the postoperative period: what you can eat, when the stitches are removed, and which signs should be a cause for concern. Find out who will manage your care after surgery — the surgeon or another specialist. If the offer seems vague and the answers evasive, it's worth continuing your search.

What to keep in mind

Key takeaways

Oral surgery covers a wide range of procedures: from tooth extraction to bone grafting and soft tissue surgery. The choice of method depends on the clinical picture, and it is decided by the doctor after examination and diagnostics. Preparation for the intervention includes tests and consultations, and recovery requires following recommendations. Not all operations are equally complex, but each requires the surgeon's skill and modern equipment. Regular preventive check-ups help identify problems at an early stage, when treatment is simpler. It is important to understand that the outcome depends on many factors, including general health and how accurately the instructions are followed.

When to see a doctor

Reasons to visit a surgeon include pain, swelling, mobile teeth, difficult eruption of wisdom teeth, and injuries to the maxillofacial area. You should also seek care if there are growths in the oral cavity, long-healing ulcers, or bleeding gums. Do not postpone your visit if symptoms are worsening or interfering with normal life. The doctor will perform an examination, if necessary order an X-ray or CT scan, make a diagnosis, and propose a treatment plan. The earlier treatment is started, the higher the chance of saving the tooth and avoiding complications. In case of severe pain or significant swelling, seek help immediately.

FAQ

Dental surgery in Almaty: most frequently asked questions

You can make an appointment with an oral surgeon by calling the clinic or using the online form on the website. During your first visit, the doctor will perform an examination and prescribe the necessary treatment. If surgery is required, you will be scheduled for the procedure at a convenient time.

When choosing a surgeon, pay attention to their qualifications, experience, and patient reviews. It is important that the doctor uses modern equipment and diagnostic methods, such as cone beam computed tomography for accurate surgical planning. In our clinic, the surgeons have many years of experience and regularly improve their qualifications.

An oral surgeon and implantologist is a doctor who performs surgical procedures in the oral cavity, including tooth extraction, treatment of inflammatory processes, and placement of dental implants. They also perform bone grafting and sinus lifting when necessary. Such interventions require high precision, so we use cone beam computed tomography for diagnosis and planning. The doctor also treats cysts, abscesses, and other conditions that require surgical access.

A general dentist treats teeth: caries, pulpitis, periodontitis, and also restores teeth with fillings. A surgeon addresses problems that require surgical intervention: tooth extraction, opening abscesses, and implantation. These specialists often work together: the general dentist treats, while the surgeon extracts or places implants.

If you have tooth pain and do not know whether the tooth can be saved, start with a general dentist. They will perform diagnostics and, if necessary, refer you to a surgeon. If the tooth is already destroyed and cannot be treated, the surgeon will remove it. In our clinic, you can get a consultation with both specialists.

The price of surgical treatment depends on the complexity of the operation, the need for additional procedures (for example, bone grafting), the materials used, and anesthesia. The doctor will determine the exact cost after examination and diagnostics. You can view current prices in the price section on our website.

Oral surgery is available in many clinics. It is important to choose a clinic with experienced surgeons and modern equipment. Our clinic in Almaty offers a full range of surgical services, including implantation and extraction of teeth of any complexity.

The cost of surgical services depends on the type of procedure: a simple tooth extraction costs less than a complex extraction or implant placement. The price also includes anesthesia and postoperative follow-up. The exact amount will be provided by the doctor after the examination. You can review the prices in the price list on the page.

Information about job openings is usually posted on job search websites or on the clinic's official website. You can send your resume to our clinic if you would like to work as part of a team of professionals. We consider candidates with the relevant education and experience.

Discount coupons for surgical services may be available on coupon websites. However, we recommend contacting the clinic directly: we often have promotions and special offers. You can get more information by phone or on the website.

Our whole family has been treated here, and we're happy with the results — there's nothing to complain about. I signed my family up here too (we had a good laugh about it at home afterwards). They told us the prices upfront, and nothing was added at the end. The contract and receipt were provided without us having to ask. The receptionist called back when she promised to. I'm one of those people who asks again three times — they were patient with me.

At another clinic they said a tooth with a cyst could only be extracted. Here they suggested a root resection and explained under what conditions it would work. The tooth is still in place two years later.
