All-on-6 Implantation in Almaty: What It Is, Stages, Advantages

All-on-6 is a method of restoring all teeth on six implants. It is suitable for patients with complete tooth loss or when chewing is no longer possible. The dentist assesses the condition of the bone and determines whether implant placement is feasible. Treatment takes several months and includes diagnostics, implant placement, and fixation of the prosthesis. Maintenance requires regular oral hygiene. The implant placement itself takes about 20 minutes. The visit is longer: before surgery — imaging and planning, afterward — a follow-up check.

The warranty applies if you come for free check-ups twice a year and get professional hygiene once a year, follow the doctor's recommendations and do not treat the same tooth at another clinic. The warranty does not cover trauma or loss of the restoration through carelessness. The warranty means free redoing or replacement of the work at the clinic's expense; the term and conditions are written in the treatment contract.

How much does All-on-6 implantation cost in Almaty

The cost of All-on-6 implantation depends on many factors: the condition of the bone tissue, the chosen implant system, and the need for additional procedures. An exact price can only be determined after an examination and diagnostics. We invite you to a consultation, where the dentist will draw up a treatment plan and calculate the cost. Our turnkey package includes the implant, surgery with anesthesia and sutures, a healing abutment, an abutment, and a permanent crown. Follow-up check-ups during the healing period are included in the price. Bone grafting and sinus lifting are not needed by everyone: if they are required, the dentist informs you before treatment begins, not along the way.

How does All-on-6 differ from All-on-4?

The All-on-4 and All-on-6 protocols solve a similar problem — restoring a full dental arch on implants. The difference lies in the number of supports and in how the chewing load is distributed. The choice depends on the condition of the bone and the clinical picture.

Number of supports and load distribution

All-on-4 uses four implants: two anterior and two angled posterior. All-on-6 adds two more supports in the posterior regions. More contact points mean a more even load on the bone. With six implants, the prosthesis is more rigidly fixed and there is less mobility under pressure. However, placing six supports requires a larger volume of bone tissue. If the bone is atrophied, the sixth implant may simply not fit. In that case, four supports are chosen. The doctor decides after a CT scan: they can see the width and height of the bone in each region of the jaw, as well as the location of nerves and sinuses. With All-on-4, angled implants bypass areas of atrophy, while with All-on-6, straight supports are more often used. The service life of both protocols is comparable if the biomechanics are maintained.

How long does treatment take

The duration of the All-on-6 protocol depends on the condition of the bone and the treatment plan. On average, the journey from examination to fixation of the permanent prosthesis takes several months.

Main stages and their duration

  1. Examination and planning: the dentist assesses the condition of the oral cavity, performs a CT scan, and selects the restoration. This stage takes one to three days.
  2. Preparation: dental treatment, tooth root removal, and, if necessary, bone grafting. The duration depends on the extent of the intervention and may take from a week to several months.
  3. Implant placement: the surgery lasts about two to three hours. Afterwards, a temporary prosthesis is placed, which is worn until the implants have fully integrated.
  4. Osseointegration: the implants fuse with the bone over four to six months. During this period, a temporary restoration is worn to ensure even load distribution.
  5. Permanent prosthesis: after healing, impressions are taken, and the permanent restoration is fabricated and fixed in place. This stage takes two to three weeks.

What the timelines depend on

The key factor is the condition of the bone tissue. If its volume is sufficient, implants are placed immediately after tooth extraction, and the overall treatment time is shortened. In cases of bone atrophy, bone grafting or a sinus lift is required first, which adds three to six months. The choice of protocol also matters: with delayed loading, implants are loaded after several months, while with immediate loading, a temporary prosthesis is fixed on the day of surgery. The doctor determines the approach after examination, so the exact timeline can only be given after diagnosis. The number of teeth to be extracted also matters: if they are severely damaged, sanitation is performed first, and implant placement is postponed until the sockets heal. Overall health also affects the timeline: with uncontrolled diabetes or bleeding disorders, osseointegration proceeds more slowly, and the doctor allows additional time. The type of implant system chosen is also important: some manufacturers have ready-made solutions for immediate loading, which shortens the waiting period. The final treatment plan is made after a CT scan and consultation with the prosthodontist who will fabricate the prosthesis.

What equipment is used?

Implantation supported by six implants requires precision at every stage. Diagnosis, planning, and placement rely on devices that reduce the share of manual work and subjective decisions.

Computed tomography and 3D planning

A cone beam CT scanner provides layered images of the jaws with accuracy down to fractions of a millimeter. With its help, the doctor assesses bone volume, the location of the mandibular nerve, and the maxillary sinuses. Based on this data, the future structure is modeled in specialized software: the points for implant placement, their angle, and depth are selected. The virtual model allows seeing in advance how the chewing load will be distributed and avoiding areas with insufficient tissue density. For the patient, this means a reduced risk of damage to anatomical structures. The clinic uses a cone beam CT scanner, and planning is carried out in a digital environment. The scan is taken before surgery and, if necessary, repeated after implant placement to assess their position. Modern software allows combining CT data with smile photographs and dental arch models, which helps predict the aesthetic result. The doctor can show the patient the future smile even before the intervention begins and discuss possible options if something in the plan is not satisfactory. Such preparation reduces uncertainty and makes the course of surgery predictable.

Surgical guides and navigation

  • Surgical guide: a custom 3D-printed overlay for the dental arch. It has holes that guide the drill precisely according to the plan, preventing deviation from the predetermined angle. The guide is fixed on the adjacent teeth or temporary supports, so it does not shift during drilling.
  • Navigation: the system tracks the position of instruments in real time and displays it on a screen. The dentist can see the depth and angle of drilling, which is especially important when working near a nerve or sinus. Navigation helps adjust the plan if the patient's anatomy differs from the CT scan data.
  • Intraoral scanner: replaces impression materials. The 3Shape scanner captures the relief of the gums and teeth within a few minutes, creating an accurate digital model for prosthesis fabrication. Scanning is more comfortable than traditional impressions, especially for patients with a gag reflex, and does not require retraction cords.
  • Microscope: optics with 25× magnification allows visualization of the smallest details of the surgical field. In dentistry, it is used for preparation and checking the fit of restorations, which reduces the risk of errors. The microscope is also used for root canal treatment and detecting cracks in roots, if necessary.

How to prepare for surgery

Preparation for All-on-6 implantation begins long before the operating room. It determines how the intervention will proceed and how the bone will heal. We explain what the patient needs to do.

Examination and tests

  • Cone beam CT scan: the scan shows bone volume and density and the position of the sinuses and nerves. The dentist uses it to plan implant placement. The scan takes a few minutes and is performed on the day of the consultation.
  • Blood tests: complete blood count, glucose, coagulation panel, HIV, hepatitis. Results are valid for about two weeks. Blood is drawn on an empty stomach, and alcohol and fatty foods should be avoided for 24 hours beforehand. If results are abnormal, the dentist may order additional tests.
  • Consultation with other specialists: for chronic conditions, a report from an internist, endocrinologist, or cardiologist confirming that surgery is safe. For example, with diabetes it is important that glycated hemoglobin levels are within the target range. Medical certificates are valid for one month.
  • Professional dental cleaning: tartar and plaque are removed to reduce the risk of inflammation around the implants after placement. The cleaning is done a few days before surgery so that the gums have time to heal. Ultrasound and the Air Flow device gently remove deposits without damaging the enamel.
  • Treatment of cavities and inflammation: the mouth is cleared of infection before the procedure so that it does not spread to the new restorations. Cavities are filled, pulpitis and periodontitis are treated, and loose teeth that cannot be saved are removed. This reduces the bacterial load in the mouth.

What to do a few days before the procedure

A few days before surgery, the doctor provides a list of recommendations. Usually, 7–10 days before, blood-thinning medications are discontinued — in agreement with the attending physician. 2–3 days before, alcohol and smoking are stopped: nicotine constricts blood vessels, which impairs healing. On the day of surgery, a light breakfast is needed if no other regimen has been prescribed. In the morning, teeth are brushed thoroughly, but mouthwash is not used. Clothing should be loose, with short sleeves or the ability to roll up a sleeve — for intravenous access. Bring a passport and recent test results. It is worth arranging in advance who will drive you home after the intervention, because driving on the day of the procedure is not advisable. It is also useful to avoid intense physical activity for 24 hours before surgery and to get a good night's sleep. If antibiotics are prescribed, they are taken according to the schedule, starting the day before the procedure. Women are not recommended to schedule surgery during menstruation, as blood clotting is reduced during those days. The doctor will give individual instructions that depend on the scope of the intervention and health status.

How to care for teeth after prosthetics?

Once a fixed implant-supported prosthesis is placed, your care routine changes. You will need to learn to use new tools and adjust some of your habits. This determines how long the restoration will last and whether your gums stay healthy.

Daily hygiene and a water flosser

  • Toothbrush: use a soft-bristled brush, brushing twice a day, paying attention to the gum line and all surfaces of the prosthesis. Use sweeping motions, from the gum toward the edge of the tooth, so as not to injure the mucosa.
  • Oral irrigator: delivers a pressurized stream of water, flushing out food debris from under the dental bridge and massaging the gums. Use 1–2 times a day, starting at the lowest power setting. An antiseptic solution can be added to the water if recommended by your dentist.
  • Interdental brushes: interdental brushes help clean the space between the prosthesis and the gum that a toothbrush cannot reach. The size is selected by the dentist: too small will not reach the plaque, while too large may injure the gum. Replace the interdental brush once a week.
  • Mouthwash: used as recommended by your dentist, usually with an antiseptic or herbal ingredients, for additional protection. It does not replace mechanical cleaning, but only supplements it. Use after meals or before bedtime, without diluting with water, unless otherwise stated on the packaging.
  • Dental floss: for caring for the prosthesis, use a special floss with a stiff end to gently clean the adjacent areas. Superfloss has a thickened end on one side that allows the floss to be threaded under the bridge. Movements should be smooth, without jerking, so as not to damage the gum.

Professional cleaning and check-ups

Even with thorough home care, professional cleanings are needed every six months. The dentist removes soft and hard deposits that a toothbrush and water flosser cannot eliminate. At the same time, the condition of the prosthesis, its stability, and the gum response are checked. Regular checkups make it possible to spot signs of inflammation or implant loosening early. Your dentist sets the interval between visits: it may be shorter if you are prone to plaque buildup or have gum disease. These visits should not be skipped — advanced inflammation can lead to implant loss. During the appointment, the bone is also assessed on an X-ray, which is taken once a year or more often if there are concerns. The dentist checks whether the prosthesis is mobile and, if needed, tightens the fixation screws. Polishing the prosthesis and removing pigment stains help preserve its appearance. If you notice bleeding, swelling, or bad breath, do not wait until your scheduled visit. A professional cleaning takes about an hour and is performed using ultrasound and special pastes.

How long do implants last?

The lifespan of an All-on-6 restoration depends on both the titanium implants and the prosthesis. The implants can last for decades, while the prosthesis requires scheduled replacement. Let's look at what to consider.

Lifespan of the implants and the prosthesis

ComponentService lifeNote
Implants20–30 years or moreWith stable bone and good hygiene
Abutments5–15 yearsDepend on load and material
Metal-framework denture10–15 yearsThe framework is more durable than the veneering
Metal-free denture5–8 yearsMore fragile material
Acrylic veneering3–5 yearsReplaced when worn or chipped

What affects durability

The main factor is oral hygiene. Plaque and food debris cause inflammation of the gum around the implant, which destroys bone. If the process is not stopped, the restoration becomes loose and has to be removed. The second most important factor is regular dental checkups: the dentist checks implant stability and the condition of the prosthesis, and adjusts the bite if necessary. The third factor is the patient's habits: teeth grinding and a habit of biting hard objects create overload that shortens the lifespan. The initial condition of the bone and the quality of placement also matter: the more precisely the surgery is performed, the lower the risk of complications. Finally, the prosthesis material determines its wear: metal-ceramic withstands more than metal-free ceramic. Regular care and timely visits to the dentist help extend the lifespan of the restoration, but the exact prognosis always depends on the clinical picture.

How to choose a clinic for the procedure

Choosing a clinic for All-on-6 implantation determines the success of treatment. Approach this systematically: review the equipment, the doctors' experience, reviews, and warranties. Don't rush; weigh all the details.

What to look for when choosing

  • Doctor's experience: ask how many similar procedures the implant surgeon has performed and whether they hold certifications in complex implantology. The doctor matters more than the clinic.
  • Diagnostic equipment: having a cone-beam CT scanner and an intraoral scanner indicates precise planning. Without them, treatment is guesswork — a risk.
  • Materials and technology: find out which implant systems the clinic uses. Well-known brands with a long track record are a sign of reliability.
  • Reviews and ratings: check reviews on independent platforms. Pay attention to negative ones — how the clinic responds to criticism.
  • Warranty: ask about the warranty period for implantation. A clinic confident in its work offers a warranty of up to 5 years.

Questions worth asking the doctor

At the consultation, ask the dentist about their experience, the number of All-on-6 procedures they have performed, the implant systems they use, and the possible risks. Find out how follow-up after surgery will be handled and what the warranty covers. Ask whether the clinic has its own dental laboratory: this affects the timeline and quality of prosthetics. Don't hesitate to ask about the dentist's experience with complex cases, such as bone atrophy. The answers should be clear and well-reasoned, without vague phrases. If the dentist avoids specifics, that is a reason to think twice. Remember that choosing a clinic is an investment in your health for many years. Also ask what type of prosthesis will be made: conditionally removable or fixed, what materials it will be made of, and how it will be attached. Find out what warranties the clinic provides for the implants and the prosthesis, and what exactly the warranty service includes. It is also useful to ask how often you will need to see the dentist after treatment is completed and what procedures are included in a preventive checkup.

Key points

A brief overview of the method and its limitations.

Key takeaways

All-on-6 is a method in which a prosthesis is fixed onto six implants. It is suitable when there is enough bone tissue to place six supports but not enough for a larger number. The doctor makes the decision after a CT scan and examination. The method distributes the chewing load across six points, reducing the pressure on each one. The implants are made from biocompatible materials and integrate successfully in most cases. The prosthesis is fabricated in advance, so fixation is performed within a short time. Caring for the restoration requires regular hygiene and follow-up with a dentist. Its service life depends on the condition of the bone, hygiene, and the patient's habits. The result is assessed by the absence of prosthesis mobility and comfort when chewing. The method makes it possible to restore dental function even with significant atrophy, provided the implants can be placed in certain anatomical zones. The prosthesis is made from acrylic or zirconia, which affects aesthetics and wear resistance. It is important for the patient to understand that hygiene around the implants requires more effort than around natural teeth.

What to consider before deciding

Before the procedure, an examination is performed: cone-beam CT and assessment of bone volume. If there is not enough tissue, the doctor may suggest bone grafting or another method. It is important to discuss expectations and possible risks. Smoking and chronic diseases can affect implant integration. After implant placement, time is required for osseointegration, during which the load on the restoration is limited. Regular check-ups and professional hygiene must be planned. The decision is made by the doctor after analyzing the clinical picture. The patient must be ready to follow the recommendations. The choice of clinic should be based on the doctors' experience and the availability of modern equipment, such as a CT scanner and microscope. It is also worth clarifying which implant systems the clinic uses, whether they have certifications, and how long they have been on the market. It is also important to find out who will make the prosthesis: an in-house laboratory or an external one, as this affects timelines and the possibility of adjustments. The patient should be prepared for the final treatment plan to differ from the preliminary one if anatomical features are discovered during surgery.

Questions about All-on-6 dental implantation in Almaty

The cost of All-on-6 implantation depends on the number of implants, the type of prosthesis, the materials used, and the complexity of the clinical case. The exact price is given by the doctor after an examination and drawing up a treatment plan, since it includes diagnostics, the surgical stage, and the fabrication and fixation of the prosthesis. Current prices are listed in the pricing section on the page, where you can review the approximate cost of the service.

All-on-6 implantation is a method of restoring the dental arch in which a full fixed prosthesis is secured onto six implants. The protocol is used when there are no teeth in the jaw or when placing a larger number of implants is impossible due to bone atrophy. The restoration distributes the chewing load evenly, and the prosthesis is made from durable materials, providing comfort and aesthetics. At the clinic, the protocol is performed by implant surgeons and prosthodontists using modern equipment, including a cone-beam CT scanner for precise planning.

Indications for All-on-6 include complete absence of teeth in the upper or lower jaw, as well as significant bone atrophy when implant placement using other protocols is difficult. The method is suitable for patients with mobile or destroyed teeth that are planned for extraction, and for those who want a fixed implant-supported prosthesis. The decision to use the protocol is made by the doctor after examination and analysis of CT scan data.

Contraindications for All-on-6 include severe systemic diseases in the decompensated stage, such as uncontrolled diabetes mellitus, oncological pathologies, bleeding disorders, as well as acute inflammatory processes in the oral cavity. Pregnancy is also a temporary contraindication. The final decision is made by the doctor after examination, taking into account the general health condition and CT data. If such conditions are present, the underlying disease is treated first, and only then is implantation planned in order to reduce the risk of complications.

After All-on-6 implantation, a soft diet is recommended in the first few days: soft foods at room temperature, avoiding hard, hot, and spicy foods. As healing progresses and after the permanent prosthesis is fixed, usually after a few months, you can return to your normal diet, but avoid excessive loads such as cracking nuts. The doctor gives individual nutritional recommendations at each stage of treatment.

All-on-6 implantation uses dental implants made of titanium or its alloys, which integrate well and provide reliable support. The choice of a specific system depends on the clinical situation and the doctor's preferences, but the clinic uses proven brands that meet international standards. Detailed information about the type of implants can be obtained during a consultation with an implant surgeon.

All-on-6 implantation is carried out in several stages: first, diagnostics are performed using cone-beam computed tomography, then planning and placement of six implants. After osseointegration, which takes several months, a temporary or permanent prosthesis is fixed in place. The entire procedure is performed under local anesthesia, and sedation may be used if necessary. Treatment timelines depend on the condition of the bone tissue and other factors, which the dentist will discuss at the consultation. The implant placement itself takes about 20 minutes. The visit is longer: before surgery — imaging and planning, afterward — a follow-up check.

The placement of All-on-6 implants is performed under local anesthesia, so there is no pain during the procedure. After the anesthesia wears off, discomfort and swelling are possible, which are relieved with painkillers prescribed by the doctor. To increase comfort, the clinic may use sedation, which allows the patient to relax. Following the doctor's recommendations speeds up healing and reduces unpleasant sensations.

The service life of All-on-6 implants depends on the quality of placement, oral hygiene, and the patient's general health. With proper care and regular checkups, implants can last for decades, while prostheses require replacement after 10-15 years on average. The clinic provides a warranty of up to 5 years on implants, which confirms their quality.

Caring for an All-on-6 prosthesis includes daily brushing with a toothbrush and toothpaste, as well as using an irrigator to clean hard-to-reach areas. It is important to visit the dentist regularly for professional hygiene and monitoring of the implants. Avoid excessive loads such as biting hard objects. The doctor will give detailed care recommendations after the prosthesis is fixed, and they should be strictly followed for the longevity of the structure.

I had two implants placed, both took, and at check-ups nothing was ever missing; I got used to them in a week and forgot they were there. Shoe covers, a cup, a napkin — little things, but everything was in place. The installment plan was arranged on the spot, no running around to banks. The hallway doesn't smell like a hospital, but like something neutral, just as promised. Not painful at all. I especially liked that they don't stay silent but talk through every step.

At first I just wanted to look around and compare prices. I came on a colleague's recommendation. I'm almost embarrassed that I waited so long. What I especially liked was that they don't stay silent — they explain every step (we had a good laugh about it at home afterwards). The implant was placed in the fall, and the crown — already in the spring!

I wanted a second opinion (we laughed about it at home afterwards) — it didn't hurt at all. The only thing was that the receptionist took a long time to find my file. Rustam told me right away what to expect. The implant was placed immediately after the extraction, and the crown was fitted later!

I had two implants placed, and both took (we laughed about it at home afterwards). . . They set up my chart right away, and asked for my passport only once — I'll note that separately. A year has passed, no complaints at all, and they feel like my own teeth. My previous experience was worse, so I have something to compare it to. We went over the X-ray together on a big screen, and they showed me where there wasn't enough bone, which really won me over. The receptionist called me back when she said she would
